At check-in for your appointment, you will be required to place a credit, debit, or HSA card on file with us. In addition, all first-time patients with unmet insurance deductibles will be asked for an upfront deposit of $200. Following claim adjudication, we will apply the deposit against the exact amount of patient responsibility, as determined by your insurance, and reflect any balance in your ledger. Patients carrying insurance plans that cannot be easily validated electronically, lack required Primary Care Provider (PCP) referrals, provide limited coverage, are out-of-network or invalid, will be seen on a self-pay basis only.

We do not accept Medicaid or any Medicaid-based insurance coverage, primary or secondary.
